Name Change

Issue

My name was changed and I would like my records to reflect the new name.

How do I get my name changed on CofC records?

 

Environment

CofC records

Employees, Students, Alumni

 

Cause

Marriage, divorce, life crisis or any reason a person changes their name.

 

Resolution

A request to change your name in the College's System must be made in person at the appropriate office.

 

Employees:

To change your name on your personnel and payroll records, present your Social Security Card that reflects your new name to Human Resources located in the Lightsey Center Basement, Room 036. Federal regulations require employers to validate that each employee's social security number and name on file exactly matches the social security number and name which appear on the individual's social security card to ensure proper tax reporting.

If name change is due to marriage/divorce a newly completely W-4 should be submitted.

Once your name has been updated in Banner by HR, you may submit the following form to have an alias email account created showing your new name: Email Request Form

 

Student Employees:

To change your name on your personnel, payroll and student records, present your Social Security Card that reflects your new name to the Career Center located in the Lightsey Center, Room 216.

 

Students:

To change your name on your student records, present your Social Security Card that reflects your new name to the Registrar’s Office located in the Lightsey Center, Room 281.

 

Applicants

The Office of Admissions will make any necessary record changes for applicants. Contact MyCharlestonApplicant@cofc.edu or 843-953-5670.

 

Alumni:

To change your name on your Alumni records, follow this link http://alumni.cofc.edu/NetCommunity/?pid=705. This will not change your Student Records. To change your name with Student Records see the information listed above for Students.

 

Obtain A Social Security Card

 A new social security card can be obtained through your local Social Security Administration Office. Please bring an original legal document reflecting your new name to support your request for the name change. Examples of accepted legal documents include: Driver's license, Marriage or divorce record, Military records and Passport.

Social Security Online  allows you to update your Social Security Card information.

Employees: Contact Human Resources or Submit an Email Request Form to IT to have an email alias created using your new name.

Students and Alumni: Contact Registrar

People will not be able to locate their CWID or MyCharleston login if their birthdate or SSN are incorrect. They must submit a correction form to the appropriate office to get it corrected.
